We are looking for a compassionate and enthusiastic Band 4 Rehabilitation Assistant to join our friendly and supportive interdisciplinary team at the Swindon Intermediate Care Centre (SWICC). As a Rehabilitation Assistant, you will work closely with our Occupational Therapists, Physiotherapists, Nurses, and other team members to support the planning and delivery of personalised rehabilitation programmes. You'll play an important part in helping individuals regain their independence, dignity, and confidence following illness or injury.

Main duties
As a valued member of the interdisciplinary team at Swindon Intermediate Care Centre, you will work under the guidance of therapists (Occupational Therapists, Physiotherapists, and Speech and Language Therapists) to support patients on their rehabilitation journey. Your role will focus on helping individuals regain independence in daily living through tailored therapy programmes.
* Assist therapists in delivering personalised rehabilitation programmes, both one-on-one and in groups, supporting patients in exercises, self-care, meal preparation, and mobility.
* Provide encouragement and compassionate communication to motivate patients and support their families and carers.
* Help with basic assessments and report changes in patient progress or behaviour to the therapy team.
* Ensure the smooth running of the department by managing equipment, supporting infection control protocols, maintaining accurate records, and ordering supplies.
* Take initiative in managing your time across varied tasks, helping to keep the service running efficiently each day.
* Contribute to the training and support of newer assistants and volunteers, sharing knowledge and promoting best practice.
* Participate in team discussions, service improvements, staff development programmes, and Trust audits.
* Maintain up-to-date documentation and record your work accurately using SystmOne.

* To carry out general clerical, administrative and departmental duties in support of clinical work and at the direction of the therapists and any other duties delegated by senior staff.

* Experience of a previous role involving various modes of communication with a variety of people
* A knowledge of what the job entails

* NVQ or equivalent in related area
* Experience in care delivery

* Previous experience in at least 1 of the following: Physiotherapy Assistant, Occupational Therapy Assistant, Rehabilitation Assistant, Speech and Language Therapy Assistant.
* Experience of working in the NHS.
* Training in Rehabilitation competencies or equivalent base of theoretical knowledge
* Knowledge of Stroke
