Overview

You will be part of a MDT team in which you will receive clinical and managerial support, aligned to social care professionals in reflection of the NHS 10 year plan. You will be responsible for delivering quality care to a designated caseload of people in the place that they call home, following a care plan devised by the community nursing staff you will be working alongside. You will be asked to feedback concise information in a daily handover to ensure changing needs are met and risks escalated. The successful candidate will be expected to be flexible and assist colleagues to cover service needs, which will include working across the wider integrated team. Access to a car in order to carry out service duties is essential. A full induction programme will be provided.

Location: Mid Bedfordshire (Ampthill, Shefford/Biggleswade). We have posts available to work alongside the Biggleswade community nursing team delivering care to people living in Biggleswade, Sandy, Potton and surrounding villages, and the Ampthill Community nursing team delivering care to people in Flitwick, Barton and Woburn and the surrounding Mid West Beds villages.

Working pattern: The Community Nursing Service operates between 08:00 and 21:00 hours with a variety of rotational shifts including weekends, evenings and Bank Holidays. Part-time posts would be considered.


Responsibilities

* Deliver quality care to a designated caseload in people\'s homes, following care plans and collaborating with community nursing staff.
* Provide concise feedback during daily handovers; escalate risks as required.
* Be flexible and support colleagues to cover service needs across the wider integrated team.
* Hold a current access to a car for service duties (essential).
* Participate in a full induction programme and ongoing professional development.

Qualifications and Skills

* Excellent communication, IT literacy and problem-solving skills.
* Patient-centered care approach for adults over 18 living in Bedfordshire.
* Ability to work as part of a multidisciplinary team and adapt to flexible working patterns.
