Social Work Team Service Manager - EIPH and Hospital
37 hours per week. In Bridgend, the Early Intervention and Prevention Hub acts as the front door to our integrated services for all Adults. The Hospital Social Work Team fulfils the critical link between our local hospitals and our community services to help people get home as soon as they are able. The service works to identify people’s strengths and resources and ensure they get the help they need. We are looking for someone to help us build on the success of our existing teams and create a model that delivers long‑term outcomes for the people of Bridgend.

Responsibilities
* Work across teams and organisations to deliver effective social work services for adults, bridging hospital and community services.
* Lead and manage the Hospital Social Work Team to support timely discharge and safe, appropriate ongoing care.
* Collaborate with people and their families to identify strengths and resources and ensure required help is put in place.
* Develop and implement models that deliver long‑term outcomes for service users in Bridgend.
* Provide high‑quality leadership in a busy environment and promote excellence in practice and partnership working.
Qualifications and requirements
* The ability to greet customers through the medium of Welsh is a requirement for this post.
* Protecting children, young people or adults at risk is a core responsibility of all council employees.
* Enhanced DBS check with Adults Barred List is a requirement for this post.
* A valid driving licence is a requirement for this post.
* The council's Hybrid Working Policy applies to this post, providing a framework for undertaking working hours between home and the office.
