The Social Care Practitioner role is within the Front Door Team. The team is the initial point of contact for people within the borough contacting social services for the first time. They provide advice, signposting to alternative services, onward referrals, occupational therapy equipment, and complete statutory assessments in line with the Care Act 2014.
Your role will involve:
1. Assessments and interventions with residents
2. Signposting to other services and providing advice to residents contacting the team
3. Holding and managing a caseload
This is an excellent opportunity for an enthusiastic Social Care Practitioner to join our Front Door Team. The ideal candidate will have a background or experience in working with people in a care setting or similar, a good understanding of IT including MS Office, Word, and Excel, and must hold a UK driving licence.
