Advanced Clinical Practitioner Independent Prescriber
The role involves assessing, diagnosing, planning, implementing, and evaluating treatment and care for patients with undifferentiated diagnoses. It includes conducting clinical examinations, assessing patient needs from physiological and psychological perspectives, and planning appropriate clinical care.
The practitioner will assess, diagnose, plan, implement, and evaluate interventions for patients with complex needs, proactively identifying and managing treatment plans for those at risk of long-term conditions. Managing both acute and chronic conditions, they will integrate drug- and non-drug-based treatments into management plans.
Prescribing and reviewing medications for therapeutic effectiveness, supporting patient adherence, and providing medication information are key responsibilities. The role also involves prioritizing health problems, managing urgent or emergency situations, and supporting health promotion and self-care strategies.
Additional duties include managing women's health needs, opportunistic wound care, and undertaking minor surgery within competence. Effective communication with patients and carers, including delivering bad news, is essential.
The practitioner must produce accurate records, work within guidelines and standards, and contribute to quality improvement through audits and reviews. They will also support investigations, manage risks, and ensure safety and compliance with health and safety legislation.
Leadership and personal development are emphasized, including ongoing learning, supervision, and participating in appraisals. Teamwork, delegation, risk management, and infection control are integral parts of the role.
Further responsibilities include managing long-term conditions, supporting infection prevention, contributing to quality schemes, and engaging in mandatory training and professional development. The role may involve additional duties as required.
