The Neonatal Community Nursing Team manages a caseload of neonatal babies discharged from the Neonatal Unit and Post‑Natal ward who need follow‑up support and enhanced nursing care in the community. The team operates 75hrs/week over 7 days, including bank holidays, providing home visits across North Lincolnshire, facilitating support groups to promote parental wellbeing and working closely with neonatal and maternity ward staff to ensure safe early discharge for infants requiring home oxygen or feeding and blood‑test support.
The nurses work autonomously within the larger team, with supervision from the Ward Manager and Paediatric Matron.
Requirements
* Full UK driving licence required.
* Band 6 clinical nurse placement.
Responsibilities
* Provide community follow‑up to vulnerable neonates post‑discharge.
* Assess and care for babies with chronic and acute illness once discharged home.
* Conduct home visits across North Lincolnshire.
* Facilitate support groups to maintain contact with infants and promote parental wellbeing.
* Collaborate with neonatal unit and maternity ward staff to ensure safe early discharge for infants requiring home oxygen and other ongoing support.
* Adapt shift times to meet service need and maintain high standards of autonomous practice.
Qualifications & Skills
* Band 6 clinical nursing level with expertise in neonatal care.
* Strong knowledge of neonatal assessment and management for chronic and acute conditions.
* Excellent communication skills with parents, families, and multidisciplinary teams.
* Capacity to work independently within a team and liaise effectively with ward personnel.
* Full UK driving licence.
