The post holder will assist with empowering and enabling the Long Term Care Team and Nursing Homes to ensure that patient care is delivered to the highest standards of care, utilising research and evidenced based practice, in line with Swansea Bay University Health Board (SBUHB) Nursing Policies, Procedures, NMC Code of Practice and professional standards.

Main duties of the job

Undertaking clinical practice on wound care management, working competently alongside non-registered, and pre/ post registration staff, providing supervision, teaching and assessment of competency where required.

The assessment of patients wound care needs, and the development, implementation, and evaluation of programmes of care to meet those nursing needs.

Develop, facilitate, deliver, and evaluate specialist teaching and education on wound care management to improve the quality of the patient experience in nursing homes, in accordance with a range of national objectives and organisational goals.

Be a role model within wound care management, and to work with staff in nursing homes to promote the acquisition of new and essential skills and knowledge that will enable them to provide high quality care to our patients in line with the values of the Health Board.

Support Nursing Home staff, in their personal and professional development, providing comprehensive and constructive feedback/ criticism where required, and escalating concerns to the appropriate manager.

Ensure that the principles that guide staff development reflect the Health Boards commitment to quality, safety, and patient experience, and that these are embedded into clinical practice.

Essential criteria

1. NMC Registered
2. Evidence of nursing related qualification to degree level

Desirable criteria

3. Experience of care homes
4. Teaching / assessing qualification to Postgraduate Diploma Level (Level 6) or equivalent

Essential Qualifications and Knowledge

5. Post Graduate Tissue Viability or Wound Management Course
6. Knowledge of National and local evidence-based Tissue Viability/wound care management care to Degree level course.

7. Welsh Language Skills are desirable levels 1 to 5 in understanding, speaking, reading, and writing in Welsh
8. Have an understanding of ESR.
