The social worker will provide a comprehensive service to individuals receiving care from the team, as well as to their carers and families when relevant. After assessing each person's needs, the social worker will offer support regarding the social, emotional, physical and financial impact of their condition or disability. This support will be proactive and cover a variety of issues, including access to resources, community care, employment or education, fertility treatments, childcare, transplantation, end-of-life care, and bereavement. Additionally, the social worker will assist individuals with extra vulnerabilities, such as learning disabilities, mental health issues, substance misuse, or other challenges.
