About this opportunity
We are looking to recruit Social Care Workers to join our Intermediate Care Team based at Maudes Meadow in Kendal.
The service supports residential short-term intervention, providing Intermediate Care and Discharge to Assess (D2A) beds.
Its purpose is to support recovery and maximize independence for individuals, identifying assistance required through the creation of strength-based support plans.
The service will also provide a period of assessment & recovery for people discharged from hospital and will work with wider health and social care teams to identify and meet individuals’ long-term care needs.
About the role
As a Social Care Worker, you will undertake strength-based care and support assessments with individuals and their carers, to identify tasks and assistance needed to enable activities of therapy and reablement, supporting recovery and independence.
You will prioritize and manage assessments and person-centered care plans to ensure activities are delivered within an agreed timescale, making recommendations regarding appropriate interventions, evidencing this clearly, and seeking advice and guidance as required.
You will be expected to work in ways that promote independence and enable individuals to maximize their strengths.

Note: Due to the nature of this role, there will be a need to respond to service users who may not always be accessible via public transport; therefore, there is a requirement to travel independently via a motorized vehicle.
