Mindworks Hub Clinical Practitioner (Social Worker, Nurse or Occupational Therapist)

The Mindworks Hub Clinical Practitioner will be a member of the multi‑disciplinary specialist team, providing services to children and young people aged 5-18 years and those in transition to adult services with mental health needs, emotional and behavioural problems.

Mindworks Hub East is a highly supportive, friendly and innovative team based across the East of Surrey in Redhill and Leatherhead. As part of our i-THRVE framework, this team is focused on Getting More Help and Risk Support to Children and Young people.


Responsibilities

* Provide timely assessments of children and young people with moderate to severe mental health needs, including deliberate self‑harm assessments, and arrange timely follow‑up appointments and appropriate referrals.
* Provide support, advice and education to the wider network, including the Hub Team, and build staff knowledge and skills within the alliance partnership in the care and treatment of children and young people.
* Work independently or jointly with Mindworks Hub staff to provide specialist assessments and treatment to children, adolescents and their families who require additional help or risk support.
* Conduct autonomous clinical assessments of children and young people who may have complex co‑morbidity, work alongside the wider network and provide consultations where necessary.
* Serve as an integral part of the Hub duty rota, responding to queries from families and the wider network, including risk support.
* Collaborate with a highly experienced multi‑disciplinary team involved in the assessment and treatment of CYP with moderate to severe and complex mental health difficulties.
* Manage own caseload from referral to discharge or transfer, holding medium to high‑risk cases.
* Ensure that outcome measures for the team are implemented and audited.
* Contribute to relevant research and ensure that interventions on the pathway are evidence based.
* Oversee and complete initial assessments, facilitate warm handovers and referrals to other services, and discharge with a signed care plan regarding safety, all recorded on the electronic patient record.
