3 months contract with local authority
This role is integral to delivering effective Adult Social Care Services within an integrated team focused on safeguarding, support planning, and enhancing the wellbeing of adults with care and support needs. The position involves assessing individual needs, co-designing support plans, and ensuring their regular monitoring and review. The role requires managing a personal caseload while upholding the Department and Council's reputation and adhering to national and service procedures and guidelines.
Responsibilities:
* Manage a complex and varied caseload with appropriate supervision and support.
* Work within legislative frameworks, preparing reports and representations for court hearings.
* Make complex decisions and manage risks using professional judgment and evidence-based tools.
* Communicate effectively in complex or high-risk situations.
* Represent the department in multi-disciplinary forums and chair meetings as needed.
* Conduct direct work with service users, carers, and families following safeguarding policies.
* Assess and navigate care for individuals with eligible social care needs in collaboration with health agencies.
* Plan responses to meet individual needs with carers, advocates, and health agencies.
* Deliver services within financial resources to meet assessed needs and outcomes.
* Coordinate, monitor, and review support plans, identifying service gaps and advising management.
* Collaborate with Social Care Assessors and identify tasks in agreement with leadership.
* Undertake duties specific to the AMHP role if qualified, including participating in the Out of Hours AMHP rota.
* Facilitate student placements as a practice educator.
* Conduct capacity and best interests assessments.
* Lead safeguarding inquiries, ensuring timely completion and communication of outcomes.
* Work cooperatively within the Council and across multi-agency boundaries.
* Partner with an integrated team of allied health professionals.
* Identify and address poor practice issues, promoting inclusive practices.
* Obtain regular professional supervision for effective practice and career development.
* Engage in Performance and Development Reviews, pursuing learning opportunities and maintaining professional development records.
* Contribute to team development and participate in meetings.
* Ensure high-quality, timely reports and authorized expenditure on cases.
* Maintain accurate, lawful client information data in compliance with confidentiality laws.
