What you will do: Conduct assessments of individuals to determine their care and support needs, implementing care plans tailored to their requirements. Collaborate with other professionals and agencies to ensure that service users receive holistic and coordinated care. Maintain accurate records and documentation of all interactions and care plans as per standard operating procedures. Current registration with Social Work England. Proven experience in Social Services within the UK, working as a qualified Social Worker with substantial frontline exposure. You will undertake strengths-based assessments, support adults and carers, manage safeguarding and risk, work within the Care Act and Mental Capacity framework, and collaborate with families and partner agencies to achieve positive outcomes. You must be confident in managing competing priorities, making sound professional decisions, and maintaining accurate case recording.
