Be responsible for assessing and coordinating care for patients with complex needs, including those facing social or environmental challenges that may impact the effective delivery of nursing care. Be responsible for the effective triage and patient caseload coordination by working closely with the wider MDT. Be responsible for providing managerial and clinical supervision to both registered and unregistered staff, as well as students, including the delivery of clinical supervision sessions. To work collaboratively with colleagues, other professionals, external agencies, and stakeholders to support teams of dedicated nurses to promote optimum health, self-management, and care delegation to ensure patient s needs are met in relation to ongoing care. To provide direct patient care and to be responsible for the development of specialist care interventions and a role model to the Hub. Lead and manage the day-to-day operations of the community nursing team and to deputise in the absence of the Senior District Nurse. To support the delivery of a clinically effective service which meets the needs of individuals, families, and communities in line with local and national agenda and Trust policies and procedures. To support the provision of a cost-efficient service and the management of resources across the Hub.
