Person specification Education/Qualifications Essential criteria Higher Professional Diploma in Health and Wellbeing (NVQ/QCF - Level 3) or a recognised equivalent course Willingness to undertake further training as appropriate Desirable criteria GCSE grade A-C or equivalent in English and Maths ECDL certificate Registration with the national regulatory body (when it becomes available). PSI qualification Experience Essential criteria Experience of working in a health or social care setting Experience of working as part of a team Experience with the use of Microsoft Office and a variety of databases. Desirable criteria Experience of working as part of a team Experience with the NHS patient information database (SystmOne, Rio, CIS) Skills and Knowledge Essential criteria Knowledge of NHS Demonstrate an acceptable level of numeracy and literacy. An awareness of risk management. Knowledge of multiagency services available and processes for referral Knowledge of the effects of the ageing process and needs of vulnerable adults in relation to their health and well-being Excellent communication skills and ability to build effective relationships Ability to analyse, interpret and record data in a clear, and succinct manner Good analytical and problem-solving skills Excellent oral and written communication and presentation skills Desirable criteria Knowledge of local health needs strategies.
