Overview
The main duties of the posts are to care for individuals with complex health care needs in their own homes within the community. Successful staff will be expected to enable the individual to live a full and productive life supporting physical and psychosocial needs within her family unit. This unique opportunity allows the nurse to develop a relationship with the patient to know her individual needs and health needs to provide high, quality nursing care.

Responsibilities
* Care for a young girl transitioning from paediatric care to adult care, living in the Blaenau Gwent Area.
* Utilise knowledge and experience of working with patients with complex health needs to deliver high-quality nursing care.
Qualifications and Skills
* Health Care Support Worker (HCSW) with knowledge and experience of working with patients with complex health needs.
* A learning disability background is desirable but not essential.
* Willingness to learn additional skills related to caring for individuals with complex health needs.
