Experienced Occupational Therapist - Reablement

An experienced Occupational Therapist is required to join a busy Reablement Team within a local authority Adult Social Care service. The team focuses on supporting individuals to regain independence and reduce the need for long-term care.
Key Responsibilities
Manage a complex caseload of adult clients
Complete occupational therapy assessments and develop reablement plans
Carry out moving & handling and risk assessments
Recommend equipment, adaptations and Disabled Facilities Grants
Work collaboratively with health professionals, families and partner agencies
Provide professional support and guidance to colleagues
Maintain accurate records and reportsRequirements
Degree or Diploma in Occupational Therapy
HCPC registration
Minimum 2 years post-qualification experience
Experience working with adult clients and complex needs
Knowledge of health, housing and social care legislation
Experience with adaptations, reablement and moving & handlingAdditional Information
Enhanced DBS required
