Overview
The Cancer Services department at Frimley Park Hospital is a busy service comprised of Tumour Site Specific Multi-Disciplinary Teams. We are looking for a highly motivated and proactive individual who will virtually navigate patients through their care pathway from point of referral to treatment in accordance with agreed pathway steps and timescales. The role requires proactively identifying and resolving delays at all stages, with a strong understanding of the two week wait, 28 day, 31 day and 62 day targets. NHS experience is preferred, along with excellent IT, communication, interpersonal skills and the ability to prioritise and organise work with minimal supervision. The successful applicant will coordinate tumour site specific Multi‑Disciplinary Team meetings, track patients against Cancer Waiting Times targets, ensure Minimum Data Sets and Audits are completed, and accurately record relevant data for each patient in line with National Data requirements. The department supports continuous personal development through study days, conferences and training opportunities.
Main duties of the job
* To actively track patient progress through the patient pathway to ensure care is carried out in a timely fashion on a daily basis and follow escalation and tracking processes on the Patient Tracking Tool (PTL).
* Coordinate cross-site referrals to another internal specialty within the Trust.
* Coordinate MDT activities, including tracking patients post MDT, updating with requested investigations, referrals to tertiary centres, and general pathway progress on SCR.
* Facilitate multidisciplinary team meeting discussions and ensure cancer data is captured and recorded on Somerset.
* Ensure all cancer patients are identified and discussed at MDT meetings.
* Ensure diagnoses, investigations and treatment plans are recorded on Somerset to ensure cancer data uploaded is accurate.
