As we continue to evolve, were looking for a Social Worker who has completed ASYE and is passionate about safe, timely discharge planning and who thrives in a fast-moving, collaborative environment.
About the Role
As a Social Worker within our secure services, youll be a key driver of effective discharge planning, reduced length of stay and improved patient progression. Youll work closely with the Discharge Liaison Nurse, MDTs and external agencies to ensure every patient has a clear, coordinated pathway out of secure care.
You will:
1. Lead on social care assessments and contribute to robust discharge planning
2. Work proactively to identify, address and escalate barriers to discharge
