We are looking for a compassionate and proactive Social Prescribing Link Worker to support individuals, 18+ living in Milton Keynes, with non-medical needs by connecting them to local services, groups, and activities.
What you’ll do:
* Provide one-to-one, person-centred support
* Develop personalised wellbeing plans
* Link people to community resources
* Work with GP practices and local partners
What we’re looking for:
* Experience in health, social care, or community work
* Strong communication and organisational skills
* A genuine interest in green spaces and nature-based activities
* Empathy and a non-judgemental approach
