We are seeking a number experienced and forward-thinking General Practitioner s to take on a GP Interface Lead role, supporting the integration of primary and secondary care across Aneurin Bevan UHB. These role s are pivotal in improving patient pathways, reducing unwarranted variation, and enhancing clinical collaboration between sectors .
You will act as a clinical interface, ensuring referrals are appropriate, timely, and aligned with best practice. Working closely with secondary care C onsultants and operational teams, you will help shape a more efficient and patient-centred system, supported by a robust clinical governance framework.

The Interface GP will work as part of our multidisciplinary teams and support the delivery of effective coworking and pathway integration through;
Triage and management of Primary Care referrals to Secondary Care,
Improving communication and the transfer of learning between Primary and Secondary Care
Improving awareness and adherence to agreed clinical interface pathways
