Job Overview

Frimley Health's Plastics Skin Cancer department is seeking a highly motivated and proactive individual to join our team as a Patient Pathway Navigator. This vital role involves virtually supporting patients throughout their care journey, from the point of referral through to treatment. The successful candidate will ensure that each stage of the pathway aligns with agreed timelines and national cancer targets, including the two-week wait, 28-day, 31-day, and 62-day standards.

The ideal candidate will have prior experience working in a team-based environment and, preferably, within the NHS. A strong grasp of cancer pathway performance targets is essential, as is the ability to proactively identify and resolve delays across the patient journey. Exceptional IT proficiency, communication, and interpersonal skills are crucial, alongside the ability to effectively prioritise and manage a varied workload with minimal supervision.


Main duties of the job

1. To actively track patient’s progress through the patient pathway to ensure that care is carried out in a timely fashion on daily basis and follows escalation and tracking process on PTL (Patient Tracking Tool).
2. Specific co-ordination and actioning of process of cross site referrals to another specialty (internal within the Trust)
3. General co-ordination and actioning of MDT – eg. Tracking patients post MDT, updating with requested investigations, referrals to tertiary centres, and general pathway progress on SCR
4. To facilitate multidisciplinary team meeting discussion and ensure cancer data is captured and recorded on Somerset
5. To ensure that all cancer patients are identified and discussed at MDT meetings.
6. To ensure patients’ diagnoses, investigations, and management and treatment plans are recorded on Somerset to ensure cancer data uploaded is accurate.

Person specification

Qualifications

* Educated to NVQ level 3 or equivalent

Experience

* Experience of working in an office or similar environment
* Experience of using EPIC software

Desirable criteria

* Healthcare sector experience
* Experience of using Video Conferencing Equipment

Skills & Knowledge

* Ability to communicate effectively with staff at all levels
* Microsoft Office skills (including, Excel, Access, Word, and PowerPoint)
* Demonstrates the ability to analyse information quickly and communicates in a concise and articulate manner
* Knowledge of Medical terminology
* Knowledge of the EPIC system
* Minute taking skills
* Ability to respond quickly to changing demands and demonstrates strong skills in prioritisation and time management

Desirable criteria

* Knowledge of in-house Somerset Cancer Register as well as CRIS etc
