Job Summary

Working with the Patient Safety Incident Response Manager, you will develop, manage and support mechanisms for learning and sharing patient safety lessons from reported incident investigations and excellence in practice.

You will co‑coordinate activity on the Risk Management Database (Datix) across all modules – incidents, actions, claims, and risk registers. Through effective design, audit and training, strive to ensure uniformity and quality of data entry from across the Trust.

The post holder will be the in‑house specialist lead for Datix throughout the Trust, ensuring staff are trained and supported in use of the product; ensuring that correct data entry leads to accurate statistical reports and that the Trust is able to utilise the system to the fullest.

The post holder will act as system administrator, being responsible for evaluating system updates and overseeing installation, developing policies for controlling access and monitoring use of the system; they will liaise with the external software provider and ensure all software problems are rectified promptly.


Main duties of the job

Project manage the day‑to‑day management of Datixweb across the organisation ensuring it complies with the national requirements of external agencies (Health and Safety Executive, NHS England, MHRA).

Contribute to identifying and planning a strategy to further improve the utilisation of Datix‑web as a tool for risk management supporting good clinical governance. Be responsible for the way the system is used to ensure learning and feedback from reported incidents and near misses.

Lead on the development, initiation and delivery of training on the Risk Management principles underpinning the Datix modules (Incidents and Risk Register) and lead the first‑line support for queries. This will involve training senior management as well as frontline clinical and non‑clinical teams.

Build relationships with senior Operational Managers, Clinical Governance Leads and Datix users across the organisation to change‑manage the use of Datixweb and encourage collaborative working.

Job responsibilities

Support the Risk Management team in the delivery of business objectives in relation to effective reporting and learning systems for the organisation.

Work with the Serious Incidents Investigations & Learning Manager to develop, manage and support mechanisms for learning and sharing patient safety lessons from reported incident investigations and excellence in practice.

Co‑ordinate activity on the Risk Management Database (Datix) across all modules – incidents, actions, claims, and risk registers. Through effective design, audit and training, strive to ensure uniformity of data entry from across the Trust.

Act as system administrator, evaluating system updates, overseeing installation, developing policies for controlling access and monitoring use of the system, liaising with the external software provider and ensuring all software problems are rectified promptly.

Support the Serious Incidents Investigations and Learning Manager with the development, modification and implementation of systems for the management of serious incidents in accordance with the national framework and CQC regulations.

Work to agreed objectives but with autonomy to determine how these objectives are met.

Deputise for the Serious Incident Investigations & Learning Manager in their absence.

Ensure provision of advice, support and training to members of staff at all levels across the Trust who have access rights to the Datix system without reference to a manager.

Ensure the uploading of approximately 36,000 patient safety incidents per year to the Learning from Patient Safety Events System (LFPSE).


Person Specification


Experience – Essential

* Experience in delivering training
* Evidence of continuing professional development


Experience – Desirable

* Development of training courses in a variety of formats for mixed audiences


Skills and Behaviours – Essential

* Flexible, adaptable
* Ability to relate to staff at all levels within and outside the Trust
* Able to work as part of a team but also on own initiative
* Awareness of need for discretion and high levels of confidentiality
* Highly developed IT skills
* Working knowledge of the associated Datix modules
* Excellent organisational skills, including high level of accuracy and ability to work to tight deadlines
* Ability to liaise effectively and effect change with managers and Datix users at all levels
* Ability to analyse, investigate and interpret complex data and produce statistical reports
* Extensive use of Microsoft packages – specific knowledge of Outlook, Excel, and Word


Skills and Behaviours – Desirable

* Highly specialist knowledge of Datix database, particularly the incident reporting module, gained through experience
* Experience of delivering presentations
* Working knowledge of SQL
* Experience in incident investigation


Qualifications – Essential

* Numerate and literate to minimum of degree or equivalent level
* Knowledge of a range of IM&T areas in computing or relevant proven experience in introducing Datix applications


Qualifications – Desirable

* BSc in computing
