Your new role In this role, you will work collaboratively with adults, families, carers, health partners, and community organisations to ensure safety, wellbeing, and positive outcomes. You will apply a strength‑based approach, support complex decision‑making, and act as a key professional in safeguarding enquiries under the Care Act 2014.In this role, you will: Undertake proportionate strength‑based assessments, reviews, and interventions. Lead and contribute to safeguarding enquiries, ensuring a "Making Safeguarding Personal" approach. Work within relevant legislation including the Care Act 2014, MCA 2005, and MHA 1983. Develop clear, outcomes‑focused support plans. Provide advice on equipment, Assistive Technology, housing adaptations, and reablement services. Manage a varied caseload, including duty work and emergency responses. Work closely with multidisciplinary teams and external agencies. Maintain accurate, defensible case records.
