Overview
:
1. Hold caseload of complex case patients due to release, referred by Prison Healthcare Teams.
2. Be required to work with key prison stakeholders enabling effective coordination and transfer of service user treatment.
3. Work in partnership with other healthcare providers as part of an Integrated Health and Wellbeing model.
Key Responsibilities:
4. Work intensively with patients for 12 weeks prior to and post-release.
5. Develop relationship with patients and co-design a holistic resettlement plan that ensures continuity of care for all their healthcare needs.
6. Review personal goals, preparing patients for release (. what to expect on day one of
leaving the gate).
7. Work closely with CGL Teams, Healthcare, Prison partners and External Support
Agencies to ensure effective coordination of Release Planning.
8. Contribute to risk management processes, including prison ACCT process as
appropriate.
9. Provide accurate and up to date harm reduction advice, including delivering training
clients in the use of Naloxone.
