Summary
* The Move-On Coordinator (Hospital Discharge) is a key member of the Mulberry Junction team, dedicated to supporting individuals affected by homelessness in achieving positive and sustainable accommodation outcomes. This role is crucial in coordinating the rapid and safe transition from hospital to suitable community-based settings, ensuring that health, wellbeing, and housing needs are comprehensively addressed.
Responsibilities
* Support in-patients experiencing homelessness, including those with undetermined housing status or NRPF, to achieve safe hospital discharge.
* Conduct Move-On Assessments to identify needs and suitable accommodation options.
* Provide short-term resettlement support once clients move into new accommodation, ensuring they settle safely.
* Assist in gathering information required for Duty to Refer (DTR) investigations and hospital discharge planning.
Requirements
* Experience presenting data and information clearly and appropriately.
* Commitment to supporting people affected by homelessness with dignity and respect.
* Knowledge of the Homelessness Reduction Act (2018).
* Knowledge of the Care Act (2014).
* Understanding of the Settled Status process for EU nationals.
