Job overview

A core aim of the Urgent Community Response Team is to reduce the number of avoidable attendances to A&E and admissions to hospital, and support early discharges from hospital, providing more effective crisis care pathways closer to home in the community.

The service is a Home First service model based on the principle of supporting an acute health and social care crisis in the patient’s home environment with prompt specialist intervention to avoid unnecessary unplanned admission to hospital.

Our response time is 2 hours of receiving referrals. A timely response will also ensure people maintain their independence. It may be appropriate to arrange an admission to a community hospital bed or interim/respite stay in a care home, to avoid an admission to acute hospital in some circumstances, dependent on availability and transport.

Main duties of the job

The post holder will be a Registered Nurse (RN12-NMC) or Paramedic (HCPC) who will have significant intermediate care experience at Band 5 or 6. Must possess Clinical Assessment Skills, Minor Illness and either Non-Medical Prescriber or working towards qualifications. The post holder will demonstrate safe clinical decision making and expert care for the patients registered within the service. The post holder will be professionally and legally accountable for all aspects of care from initial history taking through to treatment. They will be responsible for patients allocated to them and will assess, manage and deliver care to meet the complex needs for those patients requiring intermediate care and primary care services. The post holder will be able to manage and support their peers, working on their own initiative and but also an effective team member along with the requirements of pre-registration students. They will be committed to working collaboratively, actively supporting and liaising with other health, social professionals and other agencies.

As senior nurse you will be clinical as well as managing shifts in the absence of Band 7

You will also have supervision responsibilities.

High Cost Area Supplement - Fringe

This post also attracts payment for High Cost Area Supplement of 5% of the basic salary (with a minimum of £1,192 to a maximum of £2,011 pro rata for part time).

Detailed job description and main responsibilities

1.To be responsible for organising and prioritising own and others workload and day to day allocation of work along with forward planning and delegation relating to service delivery.

2.To act when required as manager for the team of clinical staff which may include registered nurses and health care assistants.

3.To organise team off duty and annual leave and ensure others are not disadvantaged.

4.To ensure positive returns and mileage are completed and submitted on time.

5.To have organisational knowledge relating to trust protocols and procedures and ensure these guide staff.

6.To be able to deal with minor grievances and be guided by trust policy where appropriate.

7.To be responsible for providing accurate documentation for audit purposes.
