This role supports the provision of palliative care to children, young people, and families living with a life-limited condition across Sussex and South East Hampshire. Palliative care includes respite breaks, enabling children to have fun and make memories, 'adding life to shortened years'. The hospice delivers end-of-life care to a small number of children, working with specialist teams providing training and advice to manage children's symptoms effectively.
You will work as part of a large team comprising clinical staff, activities coordinators, and the child and family support team.
Essential criteria
* Provide a skilled assessment of the needs and resources of referred individuals.
* Active involvement in caseload management from new referrals through to allocation.
* Liaise with community and in-house nursing teams and colleagues in the Child and Family Support Team regarding referrals.
* Deliver in-person and online therapeutic interventions to individuals (children, young people, adults), couples, or family groups to support families with a child with a life-limiting condition or bereaved family members.
* Deliver group therapeutic interventions to support those experiencing complex loss.
* Work with the multi-professional team to provide appropriate support services to children, young people, and families.
* Identify and respond confidently to the changing needs of patients and families, adjusting interventions accordingly in discussion with the Team Leader.
* Provide support for families at points of crisis when appropriate.
* Ensure all work is appropriately recorded and entered into databases.
