Responsibilities:
4. Conduct comprehensive assessments of service users to determine their needs and eligibility for services.
5. Develop, implement, and review care plans in collaboration with service users and their families.
6. Coordinate and manage safe and timely discharges from hospital settings.
7. Work collaboratively with other professionals such as doctors, nurses, and therapists to ensure a holistic approach to care.
8. Advocate for the rights and needs of service users, ensuring they have access to the resources and support they require.
What we are looking for in a Social Worker:
9. Social Work England registration
10. Eligible to work in the UK
11. Hold a full UK licence
12. Post-qualified experience
13. Degree level or equivalent in Social Work
