Job Overview
Join Our Hospital at Home Team as an Occupational Therapist! Our Hospital at Home (H@H) Service is looking for an Occupational Therapist to provide expert, home‑based care that helps patients stay independent and recover safely at home.

Main Duties
* Deliver assessments and interventions across care homes, residential units, and patients’ homes.
* Work closely with a supportive multi‑disciplinary team to reduce hospital admissions and facilitate early discharges.
* Supervise and mentor junior staff and students.
* Prescribe, order, and teach the safe use of equipment.
* Build strong partnerships with colleagues, patients, families and local agencies.
Person Specification
Education/Qualification
* Diploma/Degree in Occupational Therapy
* Registration with the Health and Care Professions Council
Experience
* Relevant work experience in a variety of clinical settings.
* Evidence of up‑to‑date CPD.
* Experience working as part of an MDT.
* Use of functional and cognitive assessments.
* Evidence of previous employment as an Occupational Therapist at Band 5 grade.
* Experience working in the Care of Elderly and with Acutely unwell patients.
Skills & Knowledge
* Knowledge of current best practices in Occupational Therapy and demonstrate clinical reasoning, applying theory to practice.
* Demonstration of ability to work single‑handedly with individuals or in a team.
* Understanding of professional ethics and their application in practice.
* Adherence to national, professional, and work‑related policy.
* Health, safety, and risk awareness.
* Understanding of clinical governance and its implications for the profession.
* Good organisational skills, ability to prioritise and manage time/caseload effectively.
* Understanding of team dynamics and good interpersonal skills.
* Effective written and oral communication skills. Ability to listen and present written and verbal information concisely and logically.
* Good IT skills (word processing and internet).
Key Attributes
* Able to work both independently and with direct and indirect supervision.
* Interest in working in outpatients, wards, and community settings.
* Willingness to work flexibly at a variety of locations, travel from site to site within the service delivery within the Borough of Barnet.
* Hold a full, valid, UK driving licence and have access to a car to use for business purposes (unless you have a disability as defined by the Equality Act 2010).
