As an essential part of Adult Social Care Operations, this role requires you to bring your expertise to deliver strength-based assessments and person-centred support planning to those with complex needs. Your work will include Mental Capacity Assessments and engaging in Court of Protection duties, ensuring the highest standard of service within our community.

What you will do
* Conduct strength-based assessments to identify the needs of adult service users with learning disabilities
* Develop person-centred support plans tailored to individual needs and preferences
* Facilitate long-term support arrangements and regularly review these to ensure continued efficiency and suitability
* Carry out Mental Capacity Assessments to ensure clients' needs and rights are fully considered
* Engage in Court of Protection work, advocating for clients' best interests
* Collaborate as part of a multidisciplinary team providing efficient and personalised services to adults, carers, and families, ensuring key objectives and quality standards are met
