Overview
We are advertising an opportunity for an experienced Mental Health Nurse, Social Worker, or Occupational Therapist to work within the Community Mental Health Hub and support Transforming Care work in the Milton Keynes locality area.
Previous experience working with Autistic people is a key requirement, in addition to experience working in community mental health services. There will be opportunities to develop and deliver both one-to-one and group interventions for service users, as well as training and support to other practitioners. The role also has scope to lead on and contribute to service development relating to care for Autistic people and Neurodiversity. You will be supported to develop your leadership skills within the role to support colleagues in local mental health services undertaking Transforming Care responsibilities.

Responsibilities
* Provide one-to-one and group interventions for service users in line with Transforming Care initiatives.
* Deliver training and support to other practitioners to enhance care quality and outcomes.
* Lead on and contribute to service development related to care for Autistic people and Neurodiversity.
* Support colleagues in local mental health services to undertake responsibilities within Transforming Care.
Qualifications and Requirements
* Professional background: Mental Health Nurse, Social Worker, or Occupational Therapist with relevant registration.
* Experience working with Autistic people and in community mental health services.
* Core skills in Key Working with proven skills in Mental Health practice, including care planning and risk assessment.
* Experience utilizing the DIALOG+ approach for care planning and risk assessment.
* Knowledge of local and national Transforming Care Programme initiatives.
* Understanding of the Mental Capacity Act, Mental Health Act, Care Act, and Autism Act.
* Ability to work in partnership with Local mental health inpatient areas, Community Mental Health Hub, ALDT, NHS East of England Provider Collaborative, carers and families, and the local authority.
* Willingness to develop leadership skills and contribute to team development.
