Job Summary
We have an exciting opportunity for a non-clinical care coordinator to join our Safeguarding team in North & West Primary Care Network. You will work closely with the Senior Clinical Safeguarding Care Coordinator to ensure a responsive, high-quality safeguarding service for both children and adults. You will also support the PCN Care Home Care Coordinator in protecting our most vulnerable patients.
Main duties of the job
The successful candidate will play a key role in coordinating safeguarding information for both children and adults. You will support effective communication and collaboration across a wide range of teams, including the Primary Care Network, acute services, voluntary and community organisations, the Multi-Agency Safeguarding Hub (MASH), and the wider Integrated Care System (ICS). As a key member of the PCN Wellbeing Team, you will work closely with Care Coordinators, Social Prescribers, Health & Wellbeing Coaches, and allied health professionals.
Interviews are expected to take place during the week commencing Monday 2nd June 2025.
Key Responsibilities Will Include:
* Providing administrative support for safeguarding and Care Home processes.
* Managing safeguarding documents and records with accuracy and confidentiality.
* Ensuring clear, timely communication with partner organisations.
* Attending meetings with healthcare professionals and statutory agencies, including GPs, Primary care nurses, health visitors, school nurses, and social care teams.
* Facilitating multi-disciplinary team working by acting as a liaison between teams involved in the care of vulnerable individuals, such as the Primary Care Network team, Multi-Agency Safeguarding Hub (MASH), and the Integrated Care Board (ICB).
