Service Support Worker (Home from Hospital Service) Location: Croydon, London Salary: £22,932 (plus Inner London Weighting £3,344 if applicable) per annum pro rata Hours: 28 per week Contract: Fixed Term (until end of June 2026 with possibility to extend) Driving: A full driving licence and access to a vehicle is preferred but not mandatory Wherever you have experience or not we will equip you with the skills to thrive. By providing support and resources, you will empower people to regain their independence and confidence, all while maintaining their dignity. What will a day in the life of a Support Worker involve? Providing practical and emotional assistance to people returning from hospital or medical facilities. Accompanying service users to attend hospital appointments and other activities. Liaising with health, housing and other professionals to ensure that the needs of the service user are being consistently met. Ensuring a safe living environment for service users by undertaking risk assessments. Carrying out welfare checks to assist service users with day-today activities. Building positive relationships with service users, their families and other healthcare professionals. Ensuring all care documentation is accurate and up to date. What does it take to be a successful Support Worker? Previous experience in a similar care, preferably within a healthcare or social care setting. Compassionate and empathetic nature, with a strong desire to make a positive impact on individuals lives. Excellent communication and interpersonal skills, with the ability to work effectively within a multidisciplinary team. Knowledge of person-centred care principles and a commitment to upholding dignity and autonomy. Ability to work independently, demonstrating initiative and reliability. Flexibility to work various shifts, including evenings, weekends, and bank holidays. A full driving license and access to a vehicle is preferred but not mandatory.
