Join to apply for the Rehab Support Worker role at NHS Ayrshire & Arran. This role is part of the Integrated Community & Support Services (ICASS) in the St Andrews Locality.
An exciting opportunity has arisen for an experienced RSW to join our Intermediate Care Team. The Intermediate Care Service provides rehabilitation to the adult population within the locality, delivered to service users as in-patients, out-patients or within their own homes. The primary function of this role will be to work with Occupational Therapy, Physiotherapy & District Nursing Services to deliver therapeutic treatment programmes to service users in the ward setting or their own homes, preventing hospital admission and supporting early discharge from hospital. You will be expected to carry out ongoing treatment programmes for a mixed caseload of mainly older people.
Key responsibilities
* Work with OT, PT and District Nursing to deliver therapeutic programmes.
* Provide daily care in the community, 365 days a year.
* Carry out ongoing treatment programmes for older people.
* Support early discharge from hospital and prevent admission.
Qualifications & experience
* SVQ Level III in Health and Social Care or equivalent.
* Experience working as a support worker desirable.
* Experience working with older people desirable.
* Excellent communication, organisational skills and ability to work independently.
Working conditions
* Part-time, 15 hours flexibly over 7 days.
* May be required to work in other community hospital sites.
* Travel throughout the area essential.
* Regular supervision, PDP and supports training opportunities.
