We are expanding our Pharmacy Team at Stafford Central PCN and arelooking for a dynamic Clinical Pharmacist to join our team.

Part time (30 hrs) to work for Gnosall Surgery

The post holder will work as part of a multi-site, multi-disciplinaryteam in a patient-facing role. The post holder will take responsibility formanaging targeted areas of chronic disease management within the practice andundertake structured medication reviews to proactively manage patients withcomplex poly-pharmacy.

The post holder will provide support to the general practice staff withregards to prescription and medication queries. They will help support therepeat prescription system, deal with acute prescription requests, andmedicines reconciliation on transfer of care and systems for safer prescribing,provide expertise in clinical medicines advice while addressing both public andsocial care needs of patient in the GP practice (s).


Main duties of the job

Repeat prescribing review Practice repeat prescribing policies. Overseethe repeat prescribing reauthorisation process by reviewing patient requestsfor repeat prescriptions, reviewing medicines reaching review dates andflagging up those needing a review. Ensure patients have appropriate monitoringtests in place when required. Reduce inappropriate poly-pharmacy and wastefulprescribing and address public health and social needs of patients throughclinical medication review. This would be a level 3 medication review lookingat the patients full clinical condition e.g. blood monitoring, interface carearrangements, social isolation. Reconcile medicines following hospitaldischarge and work with patients and community pharmacists to ensure patientsreceive the medicines they need post discharge. Contribute to reductions inmedicine related hospital admissions and readmissions by supporting patients toget the best outcomes from their medicines and identifying and addressingmedicines related issues. Interface with community and hospital pharmacycolleagues and develop referral processes between primary care professionalsincluding the promotion of the repeat dispensing service.

Manage Practice formularies to improve the quality, safety and costeffectiveness of prescribing. Implement drug withdrawals and alerts e.g. MHRAaimed at improving medicines safety.


About us

Gnosall Surgery is part of a three practice Primary Care Network and we are looking for a Pharmacist to join our team. The role will be part of the ARRS scheme for our network and the successful applicant will undertake their hours predominantly at Gnosall Surgery. We are a rural practice serving a population of 8500 patients and have a good rating for CQC and a wide variety of staff groups who work closely together to provide an excellent level of service to our patients and support each other on a daily basis.

We are situated in a purpose built Health centre with a Pharmacy attached. We have excellent parking facilities.


Job responsibilities

The post holder will work aspart of a multi-disciplinary team in a patient-facing role. They will takeresponsibility for managing targeted areas of chronic disease management withinthe practice and undertake medication reviews to proactively manage patientswith complex poly-pharmacy.

Increase quality and safety ofprescribing through mechanisms such as audit and PDSA cycles.

Manage Practice formularies toimprove the quality, safety and cost effectiveness of prescribing.

Implement drug withdrawals andalerts e.g. MHRA aimed at improving medicines safety.

Work with primary careprofessionals and patients to implement NICE and other evidence-basedguidelines.

Be involved in clinics wheremedicines are the main interventions e.g. anticoagulant monitoring clinics,high risk drug monitoring.

Contribute to patient facingmulti-morbidity long term condition reviews and multi-disciplinary reviews.

Review daily Pathology resultsfor patients on known medicines (if required)

Act as a source of medicinesinformation for all of the Practice team and patients (e.g. around doses, sideeffects, adverse events, possible alternatives if stock issues).

Provide education and trainingto primary healthcare team on therapeutics and medicines optimisation.

Signposting to communitypharmacy and referring to GPs or other healthcare professionals, whereappropriate.

Undertake Patient facingdomiciliary and care home medication reviews and produce recommendations forthe nurses or GPs on prescribing and monitoring.

Work with care home staff toimprove safety of medicines ordering and administration.

The jobholder may be required atany time to undertake any work up to and/or at a level consistent with existingresponsibilities.


Person Specification


Qualifications

* MPharm (4 years) or equivalent. Membership of the General Pharmaceutical Council of GB (1 year pre-registration training). Maintains clinical and professional competence and CPD Prescribing qualification. Postgraduate qualification in clinical pharmacy (1 or 2 years) or equivalent experience.
* Membership of the Royal Pharmaceutical Council of GB.


Experience

* Experience in clinical pharmacy. Experience of working with other healthcare professionals. Experience of Clinical Audit. Experience of community and Hospital Pharmacy.
* Experience of working within a PCN in Primary Care. Minimum 5 year post-registration experience with at least 2 years in Primary Care. NHS Smoking Cessation Advisor. Experience of teaching and training to other healthcare staff. Awareness of financial management or budgetary reporting on medicines. Experience of Providing evaluated drug information.

* Familiar with EMIS Web GP computer system Project management skills Able to interpret Prescribing data IT qualification e.g. ECDL Evidence of successful project management

£49,000 to £53,000 a yearDepending on experience (Pro rata)
