An exciting opportunity has arisen for a dynamic, motivated Pharmacy Technician to join the community Medicines Management team in Swansea. The post is ideal for an enthusiastic individual who is interested in integrated working and who is committed to developing their skills to achieve the best quality care for individuals in their own homes.
Main duties of the job
The Medicines Management Facilitator (Community Services) will join an integrated, multidisciplinary community team to deliver medicines management support and education linked to care in domiciliary and intermediate settings. This will contribute to improving patient outcomes and minimising harm, which may lead to unscheduled care and admissions.
The role includes home visiting; providing medicines management interventions to patients in their own home, involving lone working without direct supervision. The post holder will assess medicines management requirements and support patients and care workers to manage medication in the community, with focus on promoting independence and an early intervention approach. The post holder will deliver medicine management training to relevant staff groups and be involved in discharge planning.

2. Evidence of professional development

4. Substantial post qualification experience
5. Experience of being involved in facilitation change to improve clinical practice
Desirable criteria
6. Experience in education and training
7. Experience and good working knowledge of clinical audit
8. Experience in both health and social settings
9. Experience of multidisciplinary and multi-agency work
Skills
Essential criteria
10. Ability to provide medicines related information to a range of health professionals, managers, social workers and care workers, overcoming potentials barriers to understanding
11. Effective interpersonal, communication, presentation and influencing skills
12. Ability to work independently and effectively with a high degree of motivation and manage own unpredictable workload
Desirable criteria
13. Ability to plan and organise complex activities requiring knowledge of medicine management across health and social care
Knowledge
Essential criteria
14. Understanding and awareness of the importance of patient centred care/ co-production of services with those that use them.
15. Specialist knowledge aquired through health related qualification such as BTEC science (pharmaceutical) or equivalent
