This is a very exciting opportunity to join a small team in the delivery of Community liver health checks. Post holder is responsible for ensuring care to the relevant client group who require Community liver health checks and liver cancer surveillance within Lancashire and South Cumbria (L&SC) Operational Delivery Network. East Lancashire Hospitals NHS trust is the host site of L&SC ODN and will be the work base. The post holder will support and assist the clinical teams in four provider trusts (East Lancashire Hospital NHS Trust, Blackpool Teaching Hospitals, Lancashire Teaching Hospitals and University Hospitals of Morecambe Bay) and other partner organisations. Liaison with primary care will also be essential as part of the ongoing national pilot to support case finding.
The post holder will act as a positive role model within the network for staff, patients, families, carers. The post holder will work closely with the ODN delivery group and provider trust clinical teams to provide the highest standards of care to patients. There will be a requirement to contribute to data collection for NHSE liver surveillance data and the community liver health check pilot.

ELHT is the host site of L&SC ODN and work base. The post holder will support and assist the Lead CNS and ANPs in four provider trusts ( East Lancashire Hospital NHS Trust, Blackpool Teaching Hospitals, Lancashire Teaching Hospitals and University Hospitals of Morecambe Bay )and third party providers such as drug and alcohol services, custodial premises, hostels and homeless centres, in assuring that high standards of quality care are delivered and maintained. The role is very rewarding, is varied day to day and covers a wide geographical area. We care for patients in a variety of settings including on a clinical van, drug services, hostels, homeless shelters, foodbanks and kitchens, prisons, probation services in all the localities as required. The post holder will support the ODN in delivering community liver health checks, ongoing assessments (diagnosis) and enrolment onto liver surveillance (if required). The post holder will play an active role in the monitoring, support, diagnosis of defined patients monitoring of their condition within specifications of clinical protocols/guidelines. We are a small, friendly and enthusiastic team consisting of a Clinical lead, Programme Manager, Project support, Clinical nurse specialists, senior support workers and administrator.
The work will provide the successful candidate with the opportunity to work across 4 hospital provider trusts and many third party organisations. The work will be varied from day to day and challenging. Full training and support will be given to the successful candidate . We would consider a band 5 looking for a development opportunity or secondment ( If approved by manager)

Essential
Essential criteria
* Registered Nurse with NMC
* significant relevant post registration nursing experience
* Evidence of proactive involvement in the mentorship and development of nurses and nursing practice
* Experience working with hard to reach patient groups /Hepatitis/ Liver disorders etc
Desirable criteria
* Post basic qualification or relevant experience within hepatology or drug and alcohol settings
* Non Medical prescriber
