Overview
The primary work location is County Hall, Taunton, with opportunities for remote work, as per manager approval. This is a permanent role, 37 hours per week (an alternative 32 hours per week permanent contract is also available). 25 days of annual leave, not including bank holidays (pro rata for part time).
What will I be doing?
We’re working to improve the lives of young people in Somerset, and you’ll be a key part of that. As a Transition Worker you will play a crucial role in supporting children and young people accessing neurodiversity pathways. This role involves gathering detailed information from new referrals to create comprehensive support plans tailored to each child’s need. You will also work closely with families, providing one-to-one support and strategies to empower parents and carers. You will work with early years settings and schools to support transition.
Your day-to-day work will involve:
* Gathering information from new referrals of young people with autism to produce a record and plan support to be implemented to ensure the best possible outcomes for the young person and their families.
* Supporting young people with autism with their communication, e.g. create symbol resources, and ensure their voices are heard.
* Working in partnership with other professionals to achieve the best outcomes for the young people and attend and contribute to various meetings. This will include Team Around the Child meetings and some support within educational settings helping smooth transition between home and school.
* Providing targeted autism telephone support and advice to families and education settings through specialist call backs.
* Providing one-to-one work within the family home to empower parents/carers with suggestions and strategies for their young people. This will include home visits to suit the specific needs of the family.
Experience and qualifications
We offer ongoing support, training and guidance to help you be the best you can be. It will really help if you have:
* A relevant qualification in education, social work, or a related field.
* Training in autism awareness and support strategies.
* Certification in safeguarding children and First Aid.
* Excellent communication and interpersonal skills, with the ability to work independently whilst managing a caseload.
* Strong organisational and time management skills with the ability to create and implement effective support plans.

Anything else I should know?
The salary for the Transition Worker is Grade 13, ranging from £25,989 to £28,142 per annum for 37 hours per week (£22,476 to £24,339 per annum for 32 hours per week).
