About the Role
The Home First Occupational Therapy Team has recently expanded to meet increased demand for support around hospital discharges, active recovery, and reablement services. We are seeking an experienced full-time Occupational Therapist to join this busy and growing team.
You will work closely with partner organisations across health and community services, building strong collaborative relationships that promote positive health and care outcomes.
The OT team plays a crucial role within the wider Home First Social Care Team and operates across the whole county. Each team member maintains links with all three geographic clusters, so the successful candidate must be willing and able to complete home visits throughout Dorset. The position is based at County Hall, Dorchester, with a combination of office-based and home working available.
Responsibilities
* Supporting safe and timely hospital discharges
* Assessing needs and delivering active recovery and reablement packages
* Collaborating with health and community partners to optimise outcomes
* Providing high-quality, person-centred occupational therapy interventions across the county
Required Skills & Qualifications
* Enhanced Adults DBS (can be processed by us)
* Eligibility to work in the UK
* SWE (HCPC) Registration
* Degree in Social Work or equivalent
* Minimum of 3 years’ post-qualification experience in the UK
* Access to a car and willingness to travel
* Strong knowledge of theoretical frameworks, practice models, research-based practice, and psychological and human development theories
