Are you an experienced administrator/co-ordinator who is effective in a fast paced, stressful environment? Do you possess excellent organisational and time management skills? If you also have the ability to interpret medical information and terms and have previous experience of using a wide range of computer programmes then we'd love to hear from you! As a full-time, permanent Admissions and Discharge Co-ordinator based at our Platters Farm Lodge home in Rainham, Kent you will manage the administration and discharge procedures and documentation of guests into or out of the home. This role is being offered as 40 hours per week (Monday to Friday: 08:00 - 17:00) with a salary of £27,000 per annum. As Admissions & Discharge Co-ordinator, you will: enter guest information into the computer system, creating an initial care summary, risk assessment and in-depth care plan greet guests on arrival and book them onto the service assist guests with transporting or transferring them to their rooms during the meet and greet have regular contact with the guests during the whole time they are with us ensuring that all elements of the guest experience is positive ensure compliance with all legal, regulatory and best practice guidelines, making sure all risks are managed and issues identified and reported in a proactive manner. Platters Farm Lodge offers short breaks, respite, rehabilitation, and long-stay facility for adults in Rainham, Medway. Individuals can live at Platters Farm Lodge on a long-term basis. The service also provides people with dementia related care and support. For some people, the fun and nurturing environment of Platters Farm Lodge is their permanent home. The service is vibrant, socially warm, and inviting for the individual and their family and friends. Our ideal Admissions & Discharge Co-ordinator will: have the Care Certificate (or be willing to obtain this) have the ability to communicate clearly and appropriately, both in oral and written form with all stakeholders be able to work as part of a team and with other stakeholders in a collaborative and engaging way be committed to your own continuing professional and personal development Although not essential, it would be an advantage if you have: previous experience of care planning direct or indirect experience of supporting those with disabilities a Level 3 qualification in Health and Social Care
