The Hospital Social Care team is pioneering a strengths-based approach and provides support to customers and carers within a fast-paced hospital environment. We operate across three hospital sites in Warwickshire and Coventry and are dedicated and supportive teams aiming to respond quickly and efficiently with robust assessments and care and support planning.
Team composition includes Social Workers, Social Care Workers, Admin, Trusted Assessors, Team Managers & Senior Managers. The work aligns with WCC’s aim to make Warwickshire the best it can be for everyone.
About the role
Joining our established Hospital Team, you’ll provide a social work service to a highly professional standard, manage a caseload, and travel within the locality of North and South Warwickshire as required. You’ll work with older adults, their families and unpaid carers, undertake and record robust assessments, and contribute to care and support planning. You’ll communicate with a wide range of customers, communities, health partners and agencies, and work on your own initiative in pressurised situations. Team members are supported by colleagues and Team Managers who provide regular supervision and day-to-day support. Regular training is available from Warwickshire County Council.
Responsibilities
* Undertake and record robust assessments and care and support planning.
* Manage a caseload and travel across Warwickshire as needed, focusing on older adults and their families and carers.
* Communicate with customers, communities, health partners and agencies; work on your own initiative in pressurised situations.
* Participate in team duties, attend team meetings, and engage in meetings determined by the Team Manager and Service Manager.
* Benefit from regular supervision and training provided by Warwickshire County Council.
Key requirements
* Professional Social Work qualification and registration with the Health and Care Professions Council (HCPC).
* Ability to work 5 days over 7 with a 4-week weekend rotation; candidates should be prepared to work weekends.
* Ability to work closely with the Multidisciplinary Health & Social Care team.
* Participation in team duties, meetings, and other meetings as required.
* Driver’s licence is essential for post-discharge reviews, especially in care homes.
* Dynamic, strength-based approach and ability to adapt to new ways of working.
* Ability to make difficult decisions and work to tight deadlines.
* Adult social care experience and good IT skills.
* Hospital Social Care experience is desirable; willingness to learn in a fast-paced environment.
* Familiarity with the Mosaic IT System used in Warwickshire.
