The Home First service integrates nurses, paramedics, therapists, health and therapy assistants to collaborate with the acute Multi-Disciplinary Team & Transfer of Care Hubs, promoting efficient hospital discharges to patients' homes.
Our multidisciplinary teams operate flexibly to provide a 7-day service, 365 days a year, between 8am and 8pm. Patients discharged into Home First receive step-down intermediate care, involving community-based assessments and interventions at home to support recovery and health monitoring post-discharge, aiming to prevent re-admission. Where needed, Home First offers acute reablement for up to 10 days, helping individuals achieve functional goals and work towards independence in daily activities.
The service's core principle ensures recovery focuses on enabling individuals to regain as much independence as possible.
Main duties of the job
As a Health and Therapy Assistant, you will provide personal, health, and therapy care for individuals across Sussex with diverse and acute needs. Your role will be varied and engaging, supporting people to enhance their independence in daily life.
You will assist multidisciplinary colleagues in ongoing assessments and care provision, including simple clinical procedures such as taking physical observations, venepuncture, medication administration, wound care, rehabilitation, and equipment provision. Observing changes in patients' conditions and maintaining accurate records are also essential.
No prior experience is required as we will provide on-the-job training, supporting your development to achieve the Care Certificate and other competencies through our framework.
With experience and further training, there are opportunities to progress to roles such as Nursing Associate or to train as healthcare professionals like Nurses, Podiatrists, Midwives, or Therapists.
