We have an opportunity for you to join our inpatient nursing and rehabilitation teams at our stroke rehabilitation unit based at Endeavour Ward (Harmony House) in Chatham.
Responsibilities
* Work within the clinical unit at Endeavour suite stroke unit.
* Assist qualified staff by undertaking personal care duties and activities of daily living for patients.
* Assist the nursing team with all nursing elements including wound care, continence management and monitoring of patients' vital signs.
* Provide and promote a high standard of fundamental care to patients.
* Report and record patient information.
* Actively participate in patients' individual rehabilitation programmes under the guidance of occupational therapy and physiotherapy professionals.
* Work autonomously as well as part of the team.
* Exercise good clinical practice and assist senior staff to achieve high morale on the ward.
* Maintain effective communication with colleagues, members of the multi‑disciplinary team, and the unit manager/sister.
* Participate in ward/unit meetings.
* Act courteously and professionally towards patients and colleagues.
* Attend training courses/sessions required by the remit of the post.
* Commit to continued education and personal development.
* Demonstrate empathy and motivate patients to take part in rehabilitation programmes.
* Recognise when to call for assistance and report changes in patient status.
* Handle written and verbal enquiries in a professional manner, dealing with them promptly and effectively.
* Participate in appropriate moving and handling of patients during nursing care tasks.
* Assist physiotherapists and occupational therapists in the rehabilitation of patients using a range of techniques.
* Implement individual patient care for a group of patients as part of the team of healthcare assistants.
* Support staff working with you to achieve the agreed standards of care within the unit.
* Ensure the patients' needs are met as identified in the care plan.
* Undertake prescribed nursing care/rehabilitation with patients independently.
* Report areas where care has not been delivered to the set standards.
* Encourage patients and their carers through empathy, sensitivity and good interpersonal skills.
* Maintain patients' privacy and dignity at all times.
* Identify problems that may interfere with the smooth running of the unit, discuss solutions with senior staff, and put forward suggestions and ideas to the unit manager/sister in charge nurse.
* Comply with organisational operational policies, procedures and values.
* Be proactive about health and safety, implementing necessary policies and reporting near misses and concerns.
* Ensure that all resources are used appropriately to provide quality care.
* Participate in orientation and induction programmes for new staff, team training and the appraisal process.
* Act in ways that support equality and value diversity, in accordance with legislation and local policies.
* Undertake mandatory and professional study days/courses to ensure evidence‑based practice.
* Maintain NVQ level knowledge and competencies by reading clinical/professional literature and research.
* Maintain a CPD portfolio and undertake relevant CPD activities.
* Ensure correct use of confidential and sensitive information in line with CALDECOTT.
* Record all interactions in relation to patient care and rehabilitation accurately and timely.
* Maintain accurate and prompt records of all clinical interventions (SOAP notes and communication to the MDT) in line with legal requirements and standards.
* Understand research and development in nursing practice and provide information to support service delivery.
* Participate in audits as required, ensuring a high standard of care delivery.
* Plan and organise own workload without direct supervision and report progress to the line manager on key tasks.
Qualifications
* NVQ Level 2 or equivalent with evidence of working towards NVQ Level 3 or a care certificate.
* Good written and verbal communication skills.
* NVQ Level 2 or equivalent (as per person specification).
Experience
* Previous care experience in inpatient settings.
