Job overview This is a fantastic opportunity for an enthusiastic and motivated pharmacy technician to join the well supported and award-winning Manchester and Trafford Local Care Organisation (LCO), community medicines optimisation service (CMOS). The service provides patient facing clinical pharmacy services, working closely with other pharmacy teams and services across Manchester Foundation Trust (MFT.) The LCO case management service aligns with the NHS 10-year plan to move care from hospital to the patient’s own home. Case management proactively supports people with long-term medical conditions at high risk of hospital admission to remain at home. The service brings together an experienced multidisciplinary team of GPs, nurses, advanced care practitioners (ACPs), pharmacists and pharmacy technicians with access to social workers, mental health practitioners and care navigators, providing support for people to stay healthy, safe and well. To apply for this post, you must be a General Pharmaceutical Council (GPhC) registered pharmacy technician with substantial experience of working within primary or secondary care. You should be confident to work independently and have excellent organisational and interpersonal skills. You must have a full UK driving licence and access to a car for work, as domiciliary visits are an important part of this role. Advert The medicines optimisation pharmacy technician is responsible for providing technical input as part of the pharmaceutical support to the case management service. Domiciliary visits to patients for medicines optimisation interventions is a key feature of this role as well as multidisciplinary (MDT) working. Other areas include: Supporting the delivery of medicines optimisation to the case management service Supporting pharmacists to establish safe working arrangements for medicines in the Community Medicines Optimisation Service Assisting in the development and implementation of pharmaceutical care plans. Liaising with pharmacists, technicians and other healthcare professionals in secondary and primary care to ensure continuity of care on patient transfer. Carrying out medication reviews with patients in area of clinical competence Identifying and act on medication errors/queries within area of competence, reporting complex issues to a medicines optimisation pharmacist as appropriate. Assisting in the management of prescribing across primary care. Providing support to other CMOS workstreams as necessary.
