Overview

Are you the nurse we are looking for to join our team? As a Practice Nurse you will join the primary care team in Tywyn Health Centre working towards delivering a new model of multi-disciplinary health care services to the people of Tywyn and surrounding areas. The team currently comprises Advanced Nurse Practitioner, Primary Care Practitioners, Practice Nurses, Advanced Pharmacists and Physiotherapist in addition to GPs and other allied health professionals.

You will undertake a wide and interesting range of nursing duties relevant to primary care and population needs, including wound care, immunisation, contraception and sexual health services, cervical screening, warfarin monitoring, lifestyle advice, assisting with minor surgery, travel health and long-term condition management. The successful applicant should already have experience of long-term conditions and working in Primary Care or General Practice.

Responsibilities

* The post holder will perform tasks prescribed and delegated by GPs and Senior Nurses, working unsupervised to Patient Group Directions, agreed protocols, within an evidence base and defined competencies.
* Undertake chronic disease management clinics to support the Quality and Outcomes Framework (QoF) including Asthma, COPD/Spirometry, Diabetes and Chronic Heart Disease.
* Identify and manage treatment plans for patients at risk of developing a long-term condition.
* Perform basic nursing tasks such as ear syringing, blood pressure monitoring, ECGs, wound care (including removal of sutures and clips) and leg ulcer management.
* Participate in vaccination and immunisation programmes; administer injections and ensure safe vaccine storage with cold chain policy adherence.
* Undertake cervical cytology screening where eligible in line with national guidelines; undertake health screening, surveillance and new-patient screens; take pathology samples (blood, urine, etc.).
* Assist GPs with minor surgery procedures and assess problems presented opportunistically by patients; deal with minor illnesses and contribute to minor illness clinics.
* Assist with hearing and communication in the team and support patients to change lifestyle as part of change-management skills.


Qualifications

* Essential criteria: Registered first level nurse with active statutory registration.
* Desirable criteria: Relevant nursing degree; membership of a professional body.


Experience

* Essential criteria: Previous GP Practice Nurse experience.
* Desirable criteria: Some community or primary care nursing experience; experience of nursing long-term conditions.


Knowledge

* Essential Criteria: Knowledge of needs of patients with long-term conditions; awareness of accountability of own role and other roles in a nurse-led service; knowledge of health promotion strategies; awareness of clinical governance issues in primary care; knowledge of patient group directions and related policy.
* Desirable criteria: Ability to identify determinants on health in the local area; knowledge of public health issues in the local area; awareness of local and national health policy; awareness of issues within the wider health economy.


Personal Qualities

* Good communicator; team player; enthusiastic.


Skills

* Clinical skills – cervical cytology, immunisation and vaccination, ear care, minor surgery.
* Change-management skills and ability to support patients to change lifestyle.
* Strong communication skills (written and verbal); ability to communicate difficult messages to patients and families; negotiation and conflict management skills; IT skills.
* Uses initiative and works well with people at all levels.


Other Requirements

* Ability to work core hours with a flexible approach to work.


Welsh Language

* Desirable criteria: Ability to speak Welsh.
