Key Responsibilities
* Provide high-quality, patient-centered care within a busy GP practice setting, including initial consultation, assessment, diagnosis, treatment, and follow-up.
* Support same-day access by triaging and clinically assessing patients in collaboration with the duty GP.
* Prescribe and manage medications in line with evidence-based practice and practice protocols.
* Promote health, contribute to the development of practice protocols and patient pathways, and mentor junior staff.
* Provide clinical leadership to the access/navigation team and support ongoing clinical and quality improvement initiatives.
* Collaborate with the multidisciplinary team to ensure continuity of care and effective decision-making.
* Participate in service development and deliver care in accordance with NICE guidelines and local policies.
Clinical Practice and Competence
The post holder is an experienced ANP who will demonstrate safe, clinical decision-making and expert care within their professional boundaries. NMC requirements for advanced practice must be met. An 80/20 split may be available to support the advanced clinical multi-professional framework for practice in England.
Essential Qualifications
* Registered first-level nurse with MSc or equivalent
* Relevant nursing/health degree
* Independent prescribing qualification
Desirable Qualifications
* Mentor/teaching qualification
* Clinical supervision training
* Long-term conditions qualification
Experience
* Working autonomously in an advanced nursing role
* Audit and research experience
* Nurse prescribing
* Experience in primary care/general practice
Knowledge, Skills and Abilities
* Advanced clinical practice skills and management of patients with complex needs
* Clinical examination, prescribing, and pharmacology appropriate to patient needs
* Clinical leadership and team-working abilities
* Effective communication, including difficult conversations and information delivery
* Change management and resource management
* Ability to interpret policies into local practice and promote quality improvement
