Responsibilities
* Undertaking assessments, reviews, and support planning under the Care Act 2014
* Completing assessments under the Mental Health Act and associated legislation
* Working collaboratively with health professionals and partner agencies
* Delivering effective, least restrictive, and person‑centred services
* Supporting a tech‑enabled approach to care and service delivery
