Continuity of Care Team Lead - 12 months fixed term opportunity, 37.5 hours/week
Are you a dynamic, passionate, and innovative midwife seeking the next step in your career? An exciting opportunity has arisen to lead our established case-loading team, Blossom & Bloom, which offers continuity of care to women and their families planning to birth in the community setting. While primarily providing low-risk care, the team supports clients wishing to birth outside of guidance within a multidisciplinary approach.
You will be involved in the next rollout of our CoC plan, liaising with Community Matrons and other CoC leads to develop three new teams next year. Your role will also include management duties for an additional team once established.
This role involves cross-site working across Hampshire Hospitals. Our Trust provides maternity services across Hampshire, including community care, Basingstoke and North Hampshire Hospital, Royal Hampshire County Hospital, Winchester, and Hampshire Hospitals Maternity Centre, Andover. The role is based at Andover Birth Centre, but travel will be required.
Main duties of the job
You will need excellent leadership skills to support the team in delivering high-quality care and be open to positive change and transformation to meet women’s needs. Supporting women choosing home or birth centre options, you will embrace plans to improve continuity of care.
You should be an enthusiastic, confident clinical practitioner with experience in various midwifery settings, ideally within the community. Your ability to influence others is crucial.

Person specification
Education & Qualifications
* Registered Midwife with a first-degree or equivalent, with evidence of CPD
* IT training or experience with hospital data systems
* NIPE trained
* Educated to Master’s level or with a leadership course
Experience
* At least 3 years’ experience in an NHS setting
* Experience with audits, research, and service improvement
* Management and leadership experience
* Community team leadership experience
* Experience with multi-lingual backgrounds
* Experience managing patient flow and site management
Skills
* Mentorship or equivalent training
* Knowledge of quality improvement methods
* Valid driving license and own transport
* Flexibility to work hours and participate in on-call care
Knowledge
* Ability to work across staff levels within the Trust
* Self-confident, assertive, proactive, self-motivated, with excellent interpersonal skills
