Reporting To: Hwb Coordinator, Swansea Carers Centre
The successful applicant will be offered the post subject to satisfactory references and a satisfactory enhanced Disclosure & Barring Service check. The post is subject to a 6-month probationary period.
Background
For 20 years, Swansea Carers Centre has supported carers and has become one of the largest carers services in Wales.
The Hwb Initiative
The Hwb aims to streamline access for carers to available services and attract new carers, including those from ethnic minorities and male carers, who may be unaware of their rights and entitlements.
The Hwb serves as a single contact point for carers in Swansea, providing initial assessments and developing support plans tailored to their needs.
Role of the Outreach Worker
The outreach worker will be part of the Carers Hwb team, working to engage carers who cannot or prefer not to access the city centre base. They will focus on engaging underrepresented groups such as ethnic minorities, male carers, and those with limited public transport access. This position is funded by a National Lottery Community Fund project.
