* Registered Nurse or Allied Health professional ( Social Worker, Physiotherapist, Occupational Therapist) with current APC
* Competent in the use oftheInteRaiassessment tool would be beneficialbut not essential

About the role
Basedat Princess Margaret Hospital, our South/West Community Services Team is alarge interdisciplinary team working with clients in the community with a goalof maintaining patient independence. We offer assessment, treatment andrehabilitation of older persons.
About You
Workingas a Clinical Assessor is about assessing clients’ needs. That could meansupporting them and their families in the community with home supports oradvocating a move to Rest Home or Hospital Level of Care facility.
Ourservice is forward thinking and we’re one of the teams in New Zealand usingInterRAI to effectively assess the needs of our patients. We have arestorative model of care enabling the people we work with to remainindependent in the community.
TheClinical Assessor is part of the interdisciplinary team and is the key link inthe care chain from the ward to community and GP services. Therefore,you’ll understand how important it is to be patient focussed and you’ll hold awho’s who of services for Older People in your head.
* Registrationwith an Allied Health professional body and a full NZ driving license is a must, as is your enthusiasm and dynamic style
* There are two 0.9FTE positions available
